Amity Health - Kojonup

Amity Health is a not-for-profit organisation providing a range of programs to support the health and well-being of our local community.

In Kojonup we can provide the following funded health programs:

Integrated Chronic Disease Care program: We provide allied health services and Care Coordinators to work with you to improve your overall health particularly if you are at risk of developing, cardiac, diabetes or respiratory conditions.

Primary Mental Health Care Services in Residential Aged Care Facilities (RACFs) program: We provide mental health services for those living with mild to moderate mental issues at the Springhaven Frail Aged Hostel.
