We know that finding the right supports can be challenging. That’s why we take the time and effort to connect you with a support worker or peer mentor that’s right for you. No more guessing who or if your supports are turning up for their shifts, that’s the Link My Plan promise. During the initial conversation we spend time getting to know you, your individual needs and goals and then create a team that you will connect with. We ensure that the supports you work with, are consistent and reliable and keep you informed and up to date with any schedule changes well ahead of time.
