The combined prevalence of diabetic foot disease in South West, Darling Downs and West Moreton HHS' catchments exceed the national average. To tackle this, we are collaborating with our community providers across these rural and remote regions. We are adopting a Hub and Spoke model to deliver inter-disciplinary acute foot disease services in a hybrid format with holistic management of our patients
