Horizon Community Services

Horizon Community Services was created for our disability community, our friends and our families who need a better way to access individualized disability support. We provide a wide range of tailored, flexible and quality disability services and support coordination for people living with disabilities of their choice and needs. We are based in Brisbane, Queensland. We are open daily from 9am -5pm

We have extensive experience in disability care and specialize in tailoring all levels of supports to ensure our participants reach their goals. Our trusted management team and support workers actively inspire and collaborate with our participants, to help them pursue more independent and fulfilling lives.

HCS Services:

Support Work / Personal Care / Assistance with Daily Activities

HCS will support you to continue living independently in your home or community:

• Bathing
• Dressing
• Oral Hygiene Routine
• Meal Preparation
• Medication Assistance
• Assistance with using public transport
• Domestic Support ( Cleaning, Gardening, Shopping and attending personal appointments)
• Assistance in manual handling or the use of equipment’s such as Hoist or other support devices
• Support with health and fitness or assistance in doing recommended exercise program prepared by allied health associated with your recovery and treatment plan.

Social and Community Participation

This service is funded by NDIS under core supports category or can also be private funded. Horizon Community Services will help you improve your ability to be actively involved in your community, and participate in social and recreational activities.

HCS staff can:

• Take you to sporting activities.
• Assist you to participate in social groups/gatherings or simply spend time with your family or friends.
• Support you in visiting libraries including assistance in choosing books/cd’s to borrow and using library equipment’s such as laptop, personal computer and printer.
• Assistance in taking you to centre based activities and accompanying you during the activity.
• Assist you to attend any class to learn new skills or enhance current skills.
• Support you with personal hygiene such as using bathroom, support with meal and snacks, and other care requirements while you are outside your home and in the community.
• Take you to innovative community activities/job and attend with you.

Community Nursing / Nursing Services

• Continence assessment and management
• Diabetes management including insulin administration
• Catheter care and management
• Wound management
• Medication management and administration including monitoring stocks and picking up prescribed drugs from the pharmacy
• Tracheostomy management
• Subcutaneous management
• Peg feeding and management
• Ventilator management.
