Refer 10 Aged Care and Dementia Services: Our approach to aged care recognises the strengths and wisdom that every older person possesses. We support consumers across all four levels of Home Care Packages, including CALD, Aboriginal, Veterans, Financial Disadvantage and Dementia specific packages; Our Enablement Project provides case management to prevent unnecessary functional decline; and Dementia Monitoring provides community-based social support for people with dementia who live alone. Mental Health Services: We provide support in a recovery model of care that recognises recovery is a whole-of-life journey that may involve periods of ‘wellness’ and ‘illness’ that change over time. Our approach involves linking individuals and supporting them to access specialist medical care when they need it, while focusing on broader life goals, social connections, physical health, spiritual health and emotional wellbeing. Proveda is the lead organisation for the Sydney North Shore and Beaches ‘Partners in Recovery’ (PIR) program, which will provide support to more than 500 people experiencing severe and persistent mental illness over three years. Disability, Carer and Respite Services: We recognise the significant role primary carers have in supporting people experiencing frailty, disability and/or illness. We provide access to respite and advisory services for people with disability and their carers. We also co-ordinate packages under the Community Support Program. Main activities relate to Aged Care Activities.
