A post-discharge program established at Royal Perth Hospital and Rockingham Hospital for clients discharging from Emergency Departments. The program runs daily from 8am-4pm, offering peer support and case management to facilitate access to primary and secondary care and community support services by addressing underlying, unmet needs to prevent hospital readmission.
