Home Care Safety Services
Clifton Community Health Services is an approved Home Care Package provider as part of the Australian Government Aged Care subsidised scheme. If you are approved for a Home Care Package, you can choose us to provide the support and services you need to live well at home such as:
- Bathing, hygiene, and grooming
- Help with impairments or continence.
- Meals and food preparation
- Nursing care
- Respite
- Access to podiatry, physiotherapy, and other therapies
- Aids to stay independent.
- Home modifications
- Cleaning, laundry, and other chores
- Home or garden maintenance
- Transport
- Social outings, shopping, and appointments
Right for you All aged care organisations deliver their services differently and charge different amounts. So, it’s important to look around and compare providers before deciding who to get your services from.
There are three main things you should consider when selecting a provider:

2 Quality, make sure you receive a good standard of care.

Meals on Wheels
What is a Meals on Wheels Service?
This Service provides home delivered meals to people who are aged, disadvantaged, frail, or younger disabled persons and their carers. The purpose of the meal service is to help people to continue to live comfortably in their own home.
How does this service operate?
The Service is managed by a community-based committee, which is elected annually. The day-to-day operations are carried out by a team of around 40 volunteers on a roster. The hours of operation are from 12.30 p.m. to 1.30 p.m. approximately.
What meals are available?
The Service provides a minimum of one meal (soup, main, dessert, yoghurt) on weekdays delivered to your home. No deliveries on Public Holidays. Extra frozen meals may be delivered on any delivery day if required. e.g., 2 extras on Friday for the weekend, or 1 extra the day before a Public Holiday. If you have any special food or dietary requirements, these will be accommodated whenever possible. This will be noted in your record. Please contact the coordinator Rodney Watton on 07 46973735.

How much will the service cost?
Meals are partly subsidised by the Australian Government, so clients only pay $9.00 per meal (May 2021). Other arrangements can be made depending on your personal circumstances but a contribution towards the cost of the meals and their delivery is generally required. This is payable fortnightly in arrears preferably by Bank Transfer or will be collected on the Monday following the end of the delivery fortnight, around lunchtime. Any price changes will be notified two weeks prior to the date of change.

Staying at Home South West QLD
The Staying at Home project is a localised version of the (Dementia Support Australia) Staying at Home Program to service the rural and remote communities of South West Queensland.
Our goal is the delivery of a program to 75 dyads (pairs: a person living with dementia and their carer) of eligible participants who are at the early stages (6-24months) of their Dementia diagnosis to develop the skills, knowledge and confidence of the carer while also demonstrating the benefits of the proactive use of high quality respite with the joint objectives of maintaining the person living with Dementia in their own home in their own community, while protecting and enhancing the wellbeing of both the Carer and the Cared for.
We wish to achieve this by providing a learning and development opportunity for both, in the provision of a three-day program were both parties stay together in a holiday style cottage in the beautiful country town of Clifton with opportunities to enjoy the regions activities and attractions all supported by a multidisciplinary team of support workers, clinicians, and allied health professionals.
